First Time Reporting spam, Am I Doing Anything Wrong

I signed up with Spamcop and sent an email with spam email as an attachment.

I get an email back stating the email I sent as an attachment was accepted for processing.  See attached picture "Spamcop1".  I then click on the link below "Use links to finish spam reporting ..."

Cling on that link takes me to a webpage.  See attached picture "Spamcop1".  At the bottom of the webpage there is a couple of boxes.  See attached picture "Spamcop3".  Am I supposed to do anything with the boxes at the bottom of the web page?  If I am then what because I have looked on the Spamcop website and see no instructions on what I am supposed to do.  Am I supposed to do anything else that I have not done to report this spam.  I get 8 or 9 emails everyday from the same place and I am tired of it.

Everything went fine but you need to click the submit butto
